چکیده

The publication on ‘Fluid resuscitation, laryngeal oedema and severe dengue’ is very interesting.[1] Saran and Azim reported a case and raised an interesting question ‘can fluid resuscitation be a risk factor for laryngeal oedema in severe dengue?’[1] Indeed, good monitoring is required for fluid therapy for severe dengue since the complication of fluid overload is possible.[2] There is no doubt that the laryngeal oedema can be a complication due to resuscitation and orotracheal intubation.[3] Nevertheless, respiratory tract bleeding and oedema can be seen in severe dengue regardless of resuscitation.[2,4] In fatal dengue cases, respiratory organs are the main affected organs.[4,5] Close monitoring of airway, breathing and circulation in any patients with severe dengue regardless of fluid therapy is needed.

Can fluid resuscitation be a risk factor for laryngeal oedema in severe dengue?

We sincerely thank the authors for their questions. We accept that bleeding in respiratory tract can be a complication of severe dengue, but it did not occur in our illustrated case.[1] It is difficult to point to a single factor that is responsible, as the disease manifests with multi-organ involvement. We suggest fluid resuscitation as an additional risk factor which is rarely discussed in th...

Restrictive versus Liberal Fluid Resuscitation in Children with Dengue Shock Syndrome: the differences in Clinical Outcomes and Hemodynamic Parameters

Background: Fluid resuscitation is the mainstay of treatment to counteract massive plasma leakage in dengue shock syndrome. We aimed to determine the differences in clinical outcomes and hemodynamic parameters of children with dengue shock syndrome post restrictive and liberal fluid resuscitation.  Materials and Methods: A retrospective observational study of pediatric patients who were betwee...

Should colloid boluses be prioritized over crystalloid boluses for the management of dengue shock syndrome in the presence of ascites and pleural effusions?

BACKGROUND Although the WHO guideline for the management of dengue fever considers the presence of ascites or pleural effusions in the diagnosis of DSS, it does not emphasize the importance of their presence when selecting fluids for resuscitation. CASE PRESENTATION We highlight three patients with DSS who received boluses of crystalloids on priority basis as recommended by WHO guidelines dur...

Fluid management for dengue in children

Dengue is a serious public health problem worldwide. Dengue shock syndrome (DSS), the severe form of dengue fever, can cause death within 12-24 hours if appropriate treatment is not promptly administered. For patients with DSS and the 30% of non-shocked dengue patients who require intravenous fluid therapy, a range of solutions is available for plasma volume support. Crystalloid solutions, suc...

Randomized comparison of oxygen mask treatment vs. nasal continuous positive airway pressure in dengue shock syndrome with acute respiratory failure.

Dengue hemorrhagic fever (DHF) is caused by dengue virus. Patients with DHF grade 3-4, termed Dengue Shock Syndrome (DSS), may develop acute respiratory failure after initial fluid resuscitation. Previously, these patients were treated with oxygen on a nasal cannula, or if necessary with tracheal intubation and mechanical ventilation. In the present prospective randomized study, we compared the...
